As Kieth stated- an attorney needs to be involved in the crafting. A simple note in your wallet wont cut it- do your loved ones a favor and spend a few hundred dollars and legally make your wishes known.
You don't want to be the next Terri Schiavo that Republicans use as a political pawn while a feeding tube plays musical chairs.

Hmmm, If everyone could make a living will and have it reimbursed (free), it could save alot of money wasted on unwanted or unneeded healthcare! Palin and Fox made this into the death panel controversy. LIES
